Scheduling your first visit

Our doctor's schedules operate by appointment only. If you would like to schedule an appointment please call our office at (217) 529-4211.

Before your first visit

If you have already scheduled an appointment, here are some things you can do before check-in:
  • Print and fill out our New Client Information form (PDF, opens in a new tab).
  • Obtain a copy of your pet's previous medical records and bring them with you to appointment.
  • Make sure you have a photo ID with you at check-in.
  • We do require a deposit for New Clients; please call for more information.

Appointment policy

  • Cancel at least 12 hours in advance by calling (217) 529-4211.
  • One courtesy no call/no show is allowed. More than one carries a charge.
To be respectful of the medical needs of other patients, please be courteous and call our office promptly if you are unable to make it for your appointment at (217) 529-4211. This allows us to reallocate our time for another patient in need of treatment. If you need to cancel your appointment, we ask that you call at least 12 hours in advance. We allow one courtesy no call/no show appointment; however, there will be a charge for more than one missed appointment.
