Thank you for choosing Laketown Animal Hospital. We appreciate your trust in us to provide the best veterinary care possible for your pet.

 

Our doctor's schedules operate by appointment only. If you would like to schedule an appointment please call our office at (217) 529-4211. If you have already scheduled an appointment, here are some things you can do before check-in:

 

  • Print and fill out our New Client Information form.

  • Obtain a copy of your pet's previous medical records and bring them with you to appointment.

  • Make sure you have a photo ID with you at check-in.

  • We do require a deposit for New Clients; please call for more information.

 

To be respectful of the medical needs of other patients, please be courteous and call our office promptly if you are unable to make it for your appointment at (217) 529-4211. This allows us to reallocate our time for another patient in need of treatment. If you need to cancel your appointment, we ask that you call at least 12 hours in advance. We allow one courtesy no call/no show appointment; however, there will be a charge for more than one missed appointment.
